I understand that all IV infusions offered by ThrIVe Infusions 2You, are elective wellness services and are not medically necessary treatments. Infusions are designed to support general wellness and vitality, not to diagnose, treat, cure, or prevent any medical condition.  

I understand that the menu is being provided privately upon my request, and its contents are not part of public advertising.

I acknowledge that no claims are being made regarding the infusions ability to diagnose, treat, cure, or prevent any medical condition. IV infusions are not a substitute for medical treatment, professional diagnosis, or prescribed medication.   

I understand that before receiving any infusion, I will be required to:  

• Complete a medical intake form that will be reviewed by a nurse and/or doctor.  
• Obtain a prescription where required for certain ingredients.   

 I understand that pricing displayed is for informational purposes only and is subject to change based on medical consultation and prescription approval. Final pricing will be confirmed after the consultation and prescription process.   

 I confirm that:  I am voluntarily requesting this information for personal educational purposes.  

• I have not been solicited or encouraged to use any IV infusion service.  
• I understand that access to the menu items offered by ThrIVe Infusions 2You, does not guarantee access to all infusion options if defined as contraindicated.
